Many sites on the Web that have forms, including tax forms. Patrons may download and print tax forms from the library's Web site. There is an E-filing FAQ to answer questions regarding filing taxes on the Internet. The list below contains a list of books and Web sites that have a variety of types of legal forms. The "Ohio Forms and Transactions" and "Personal Forms on File" are popular resources. The books listed here are not the only books in the library's collection that contain forms. The "Legal Subjects" section, for example, has books that have forms in them. Every form is not necessarily current, and in some cases multiple copies of a form may be required. Patrons may want to consult an attorney to review legal forms.
Books/Print
- Ohio Forms and Transactions: OFT / Edited by Thomas R. Swisher.
- Forms for corporations, partnerships and other business organizations, employment, personal property transactions, real property transactions, nonprofit organizations, estate planning, law office management, and other general topics. A table of laws and rules, index, and user's guide is included. The forms may be copied for personal use.

- A variety of forms and checklists used for government agencies such as the Internal Revenue Service, Small Business Adminstration, and Department of Labor, and for business record keeping. The forms should be used as a planning tool and reference material to supplement, rather than replace, government forms.

- Designed to supplement government forms and organize personal record keeping. Contains adoption, request for police report, living-together, will, prenuptial, separation, immigration, and power of attorney legal forms, among others. The binder also includes an official State of Ohio Living Will Declaration and State of Ohio Durable Power of Attorney For Health Care form. There are forms in other areas such as education, finances, social service benefits, and the military. The forms are free of copyright restriction.

Tax Forms
- Central Collection Agency, Division of Taxation - City of Cleveland
- Tax forms, tax rates, rules and regulations, services, and a FAQ are just a few of the categories of information available.
- IRS Forms and Publications
- Tax forms, instructions, publications, and notices are available to download and print. Forms and publications are searchable by date and number, and there is a topical index to forms, instructions, and publications. A FAQ answers questions regarding downloading and printing. There are links to state tax forms, information on electronic filing of taxes, and more.
- Ohio Department of Taxation
- This site has tax forms, publications, and other information for individual taxpayers, businesses, tax practitioners, and researchers. The site has a news archive pertaining to important tax developments.
- Regional Income Tax Agency
- The official Web site of "R.I.T.A.," which collects taxes from the residents of its eighty-six municipalities in the Cleveland and Columbus, Ohio areas.
- Tax Forms on the Internet
- This site from the Lakewood Public Library has a collection of federal, state, and local tax forms, and information about electronic filing of taxes in general and at the library.

- All About Forms.com
- Provides over 2,000 free legal forms in categories such as business, bankruptcy, corporations, divorce, leases, trademarks, and trusts. Further forms and legal services are available to members for a monthly fee. A useful FAQ is on the site.
- DocumentsUSA
- DocumentsUSA allows visitors to create their own legal documents online at reasonable prices. A page describes the process to create documents. Another page links to answers to frequently asked questions about different types of legal forms. State laws governing legal documents are present.
- FedForms
- Many forms from government agencies are provided. Users can search by agency and sub-agency.
- FindLaw: Forms Collections and Forms Indexes
- This Web page from FindLaw organizes forms into categories and presents an annotated list of links to Web sites containing free and fee-based legal forms
- FindForms
- Free forms for wills, eviction, bankruptcy, rent, leases, power of attorney, contracts, landlord/tenant, employment, agreements, and divorce.
- Internet Legal Research Group: ILRG Legal Forms Archive
- The ILRG Legal Forms Archive has over 1,750 business and personal forms. The type of business forms include: buying and selling, borrowing and lending, business ventures, credit and collection, employment, leases and real estate, protection of legal rights, and transfers and assignments forms. The types of personal forms include estate planning, marital and family, name change, power of attorney, and health care forms. A link to other sources of legal forms is provided if a form is not available in the Archive.

- Offers free forms for leases, partnerships, employment, sales, business, and real estate. Additional forms are available at a charge.
- LegalKitStore.com
- Cleveland Creative and SJT Enterprises offer the InfoAmerica Business and Legal Kit Series. Forms may be purchased as a packaged kit or downloaded. Descriptions exist for each of the form kits.
- Nolo Legal Forms
- Numerous legal forms are available to download for a fee. Areas include wills/estate planning, small business, employment law, landlord/tenant, real estate, and neighbor/pets.

- Forms include Small Claims Complaint, Service Request, Subpoena, Jury Demand, and more.
- Domestic Relations Forms - Cuyahoga County Court of Common Pleas
- From the Division's Web site: "These forms are offered with the prohibition that while you have a technical right to represent yourself, we recommend that you retain an attorney experienced in Domestic Relations."
- Living Will Forms
