The
principal, nurse, secretary, and members of the teaching and cafeteria staff
are trained in CPR, First Aid, and Management of Communicable
Disease. The school follows the Communicable Disease Chart, posted
in the nurse’s office, which is provided by the State of Ohio
Department of Health.
The
school requires an Emergency Authorization from each family from
which is assessed by the School Nurse. It is essential that the Emergency
Authorization Form always be current so that the family can be notified
in case of an emergency. In the event of illness/accident during
the school day, the student is sent to the nurse’s office,
if serious the parent/guardian will be notified immediately.
No student is permitted
to leave school grounds unless the parent/guardian has been contacted
and is aware that the student is on the way home. Students are signed
out in the school office.
The
School Nurse performs several annual services for student. These
include:
- Annual
height and weight recordings
- Annual
vision screening for grades K, 3, 5, 7, and 8
- Annual
hearing screening for grades K, 1, 2, 3, 5, and 7
- Screenings
for scoliosis for grades 5 – 8
- Strep
Screening Program is in effect through the Lakewood Board.
Immunizations
Immunizations for school children are requirements under Ohio Law.
Parent/guardians are responsible for presenting written evidence
that the child has the required immunizations. They include:
- 4
DPT (Diphtheria, Pertussis, Tetanus, 4th or 5th after age
4)
- 3
Polio
- 2
MMR (Measles, Mumps, Rubella, 1st after age 1, 2nd before grade
7)
- TB
Test
- Optional
immunizations include Varicella (chicken pox), Hep B, Flu,
and MCV4 (meningitis)
On the 15th day after commencement of school, any student who has
not met the above requirements will be excluded from attendance at
school. Medical authorities and school educators urge that each child
have a complete medical examination before entering school.
Blood-borne
Pathogens
The school complies with the OSHA mandate for handling of blood.
Each student is required to have a plan on file that deals with
spilled blood fluids which necessitates the use of rubber gloves
and plastic
bags. This is for the protection of all. The entire staff at SS
Cyril & Methodius
has been instructed on these mandates.
Contagious Diseases
Parent/guardians are asked to contact the school office when their
child has been diagnosed with a contagious condition, including Strep
and Staph infections, head lice, or skin conditions. When your child
has been home with an illness, it is recommended that he/she not
return to until temperature has been normal for 48 hours and the
physician gives a release. The Principal will notify parent/guardians
if a contagious outbreak occurs in school. Outbreaks of head lice
are a normal part of early school life. The school instructs students
and parents in control measures during an assembly.
When a student becomes ill at school
If a student becomes ill during the day, parent/guardians will be contacted
immediately. The parent/guardian or authorized person will be expected
to pick up the child immediately. The child will be moved to an area
away form the other children and carefully monitored for worsening
condition.
Below is a partial list of symptoms, which would prompt immediate
notification of parent/guardian:
- Temperature
of 100 F (taken by mouth method)
- Diarrhea
(more than one abnormally loose stool within a 24-hour period)
- Stiff
neck
- Vomiting
- Sore
throat or difficulty in swallowing
- Conjunctivitis
or pink eye
- Untreated
infected skin patches
- Unusual
spots or rashes
- Severe
coughing, wheezing or difficulty in breathing
- Yellowish
skin or eyes
- Evidence
of lice, scabies, or other parasitic infestation
- Complaints
of pain
Administration of Medication Policy
All
medication is to be taken to the School Office and an Administration of Medication
Form must be completed. The request should include the
medication name, dosage, schedule, etc. and must be in original child
proof container with an affixed label containing the student’s
name.
Asthma inhalers
may be self-administered after guidelines have been met and approved
by the school nurse, parent/guardian, and principal
along with Administration of Medication Form. Instruction on the
use of, and doctor’s permission must be on file in the
school office.
Only
school nurse or principal will administer medication when:
- An
Administration of Medication Form has been correctly filled
out, dated and signed by the parent/guardian and placed in
the student’s
folder.
- Medication
is in a the original container along with medication name,
dosage, schedule, etc. and must be in original
child proof
container with an affixed label containing the student’s name.
- Written
instruction from a licensed physician or dentist are secured
The
school nurse will not administer:
- Over
the counter drugs, creams or lotions such as fever reducing
medication, cough drops or syrup, lip balm, vitamins, fluoride
supplement,
sun
block, etc. without a parent’s signature
- Any
medication that is labels “as needed.” Will not
be administered without clear guidelines for administering
a dose.
- Medication
NOT in its original container
- Medication
including over the counter that is not secured by a physician’s
prescription (i.e. Tylenol™, Motrin™, cough
syrup)
Readmitting
a student to school after being ill:
The following guidelines will be used when readmitting students to
school:
- The
student must be free from symptoms for a full forty-eight
hours from fever, vomiting, and/or diarrhea
- Student
must have completed a full twenty-four hours of antibiotics
- Parent
must provide proof of a negative throat culture. If a positive
throat culture, the student may return after forty-eight
hours of prescription medication
- When
applicable, the parent/guardian must provide a doctor’s
note that the student is free of parasitic infection, including
head lice, if treated by a physician.
All student, teachers, and staff wash their hands at school after
toileting, before food preparation times, and before eating to aid
in the control of communicable disease. Student should also wash their
hands prior to Computer class. Keyboards and mice are wiped down with
antiseptic cloths and/or Isopropyl Alcohol at the end of each day.
